    Italy Tourist Visa Application Form

    For an Italian tourist visa, the standard Schengen visa application form must be filled out completely and the signature box in article 37 and on the last page must be signed by the applicant. If the applicant is a minor, the form must be signed by his/her parent or legal guardian.
