The Ukraine visa application form is a mandatory document for all short-term or long-term Ukraine visa applications; it contains the applicant's personal information, travel details and information about the purpose.
Citizens of the Republic of Türkiye are exempt from visa requirements for their travels to Ukraine for up to 90 days; for travels longer than 90 days, a long-term visa application form must be filled out to apply for a Ukraine visa.
How to Fill the Ukraine Visa Form?
The Ukraine visa form must be filled out online at https://visa.mfa.gov.ua/ and printed out and signed by the applicant.
After the applicant enters https://visa.mfa.gov.ua/ and reaches the application form, he/she must first make the necessary selections about where the application will be made, which country the person is a citizen of and the type of visa for which the application will be made; if the person does not have a visa exemption in line with the information entered, the system directs the person to the screen where the application will be made.
On the page directed by the system, if the person has an existing account, he/she must log in or create a new account.
After logging in, you can continue to the application form page by clicking the green "Fill the application form" button at the bottom of the page. When the person reaches the application form page, he/she must fill in the requested information completely and accurately.
What Information is Required in the Ukraine Visa Application Form?
The Ukraine visa application form consists of a total of 35 items and is divided into 6 main headings.
In the first heading, general information about the applicant is requested between items 1 and 9. In addition to personal information such as name and surname, the person must fill in their citizenship information and add a photograph in accordance with the requested dimensions and criteria.

In the second main heading of the application form, between items 10 and 15, information and documents are requested to verify the identity of the applicant; at this stage, information about the passport held must be provided.

In the third main heading, between articles 16 and 18, contact and residence information of the person is requested; at this stage, if there is citizenship of another country, it should be stated and the requested information regarding the citizenship of the other country should be entered.

In the fourth main heading, in items 19 and 20, the person must provide information about his/her place of employment and job, if any. If the person is a student, he/she must fill in the relevant section with information about the institution where he/she received education.

Articles 21 and 34 in the fifth main heading are related to the person's travel information. The applicant must provide information about the details of the planned travel. Detailed information such as whether the person has previously received a Ukrainian visa, the number of times he has entered Ukraine, and whether he will use Ukraine as a transit country should be shared in the relevant section.

The sixth and final stage of the application form, consisting of Article 35, is the stage where it is checked whether the application form has been completely filled out and the application is recorded.

The applicant must have filled in all sections until the last stage using letters from the Latin alphabet and without using Turkish letters such as "ü, ö, ı, ç, ğ, ş" and must check that all information is correct and complete before saving the form.

Ukraine visa form can be filled in Arabic, Chinese, German, English, Spanish, French, Ukrainian and Russian.
Can I Add a Photo to the Ukraine Visa Application Form?
Yes, a photo can be added to the Ukraine visa application form. There is a photo upload field in the first main title of the online visa application form.
The photograph to be added to the Ukraine visa application form must have been taken within the last six months, and the person's face must be clearly visible and cover 70-80% of the photograph. The Ukraine visa photograph must not have been tampered with.
Can the Ukraine Visa Form Be Filled Out Manually?
No, the Ukraine visa form cannot be filled out on paper. The application form can only be filled out online at https://visa.mfa.gov.ua.
What to Do After Filling Out the Ukraine Visa Form?
After filling out the Ukraine visa form with correct information and completely, all pages must be printed. The printed application form must be signed by the person himself or by both parents or legal guardians if the person is under 18 years of age.
After filling out the application form and preparing all the necessary documents, the person must make an appointment for the visa application at the consulate or visa application center and submit the necessary documents on the appointment day and time.
